Applying For Certification From The Pennsylvania Department Of Education

TIMING: Apply the term you expect to graduate

Follow each step of the checklist on the following pages. Read the instructions carefully before completing the required forms. The General Application for Pennsylvania Certificate – Form PDE338G may also be downloaded at www.teaching.state.pa.us

  • Complete two forms (print in capital letters; use dark blue or black ink):
    1. The General Application Form (PDE 338 G), and
    2. Part A of the College/University Verification Form (PDE 338 C).
  • Submit:
    1. U.S. Postal Money Order for $15 with the application. Do NOT write anything on the “payable to” line. Dr. Biagini will complete that part. These money orders are available only at a U.S. Post Office. Money orders from banks, cash, or personal checks will NOT be accepted.
    2. Forms PDE 338 G and PDE 338 C
    3. Copy of PA teaching certificate
    4. Copy of your Praxis test score
  • Do not fold any pages or use staples or paper clips to organize your materials. Enclose in a 9" x 12" envelope.

  • On Campus students: Make an appointment with Dr. Biagini to submit and review your application. FastTrack students, talk by telephone or send email to Dr. Biagini before you send her your application.

  • After you submit your application packet and you have been cleared for graduation at SIS (posting of final grades), your application will be approved by Dr. Biagini and delivered to the Office of Student Personnel Services in the School of Education for final signature. The University of Pittsburgh’s Certification Officer will endorse your application. This process takes a few days.

  • The Certification Officer then will send your application to the Pennsylvania Department of Education in Harrisburg. Allow a minimum of four weeks for your certificate to be prepared and mailed to you directly from the Department of Education in Harrisburg.

  • Health Certificate: Please have your physician complete Part 10 of PDE 338 G. This must be signed by a physician; the PDE will not accept the application if signed by a Nurse Practitioner, RN, or LPN. Allow enough time to accomplish this.

    2. Part A of the College/University Verification Form (PDE 338 C).
  • Submit:
