Any incident that made you feel worried or uncomfortable

The purpose of this form is for you to anonymously and safely report anything that made you feel uncomfortable within the School or at Pitt. This is designed for faculty, staff, and students. Anything you experienced that fomented negative feelings or consequences is fair game, but here are some issues of particular concern:

Racism
Sexism
Inappropriate behavior where you felt threatened mentally or physically
Noticing behavior in another person where you did not feel threatened but you are worried for that person's health and/or safety
Practices, policies, activities within the School that you feel are damaging, problematic, or hurtful
